By using REMS Wizard you confirm that:

1. You are authorized by your practice to access the EMR and the SPRAVATO® REMS portal accounts you use with it.
2. REMS Wizard prepares forms; you review every form and submit it yourself on the REMS portal. Your practice remains responsible for REMS compliance.
3. Patient information the tool reads stays on this computer. You will not send real patient data to Automation Wizard support; use a test patient for snapshots.
4. Your practice’s subscription and use are governed by the Terms of Service and Privacy Policy at automationwizard.health/legal.
